Job Title: RN Case Manager – Dignity. Compassion. Clinical Excellence.

Pay: $90,000 – $130,000 DOE

Benefits: Full Benefits Package

What the Role Involves:

• Supporting patients through complex emotional and physical transitions
  
• Providing education and reassurance to families
  
• Coordinating a high-quality care experience from admission through bereavement
  
• Collaborating with social workers, aides, and medical directors
  

We’re Looking For Someone Who:

• Holds an RN license in MN
  
• Has prior hospice, oncology, or palliative experience
  
• Balances heart-centered care with medical skill
  
• Understands the sacredness of this work and approaches it with grace
