Job Description

Summary:

Work as a non-Physician provider in a team with medical professional such a primary care physicians, nurse practitioners, and clinical staff to provide quality patient care. Provide training in innovative, evidence-based health interventions that improve quality, improve patient-centered care, and reduce costs. Develop and establish behavioral aspects of integrated care in YRMC Family Medicine Center. Provide recommendations about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. This role will also work with the Department of Graduate Medical Education to assist in many layers of resident and fellow education as directed by the Designated Institutional Official (DIO) and Chief Academic Officer.

Responsibilities:

  • Provide education to patients and families around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation. Utilizing a working knowledge of relevant medical/legal issues that impact on patient care (e.g., advance directives, child and elder abuse, mental illness, end of life issues, etc.) to make recommendations and support the health-care team, patients, and families. Participate in multi-disciplinary health care teams and provide leadership in representing clinical social work perspective/liaison with patient, family and community agencies and resources.

  • Works closely with the clinic staff to coordinate care of patient to provide consultative services to health care team members within scope of care definitions as needed. Provides Proactively identifies the needs for and conducts family meetings that result in comfort, decisions and other important outcomes.

  • Facilitates continuity of care for treatment with YRMC Case Management and Community Social Service agencies and partners with patients and families to identify needs to develop and implement a short-term treatment plan, (i.e., establishing social service goals for alleviating problems, providing supportive/ crisis counseling, assisting the patient and family in problem solving, and making community referrals.) Coordinates office discharge planning by utilizing community resources to assist patients in having the highest possible quality of life within the patient’s capacity and at the appropriate level of care upon discharge. Links patients to appropriate services upon discharge. Document assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.
  • Assists the multi-disciplinary health care team in ensuring excellent patient care services, leads quality improvement initiatives, and links team efforts with other hospital departments and with outpatient and community services.

  • Coordinates services in accordance with our payer contracts. Designs service provision to address the entire scope of care for our patients. Complete periodic observation sessions with residents in their continuity clinics and provide feedback. Assist with periodic review of resident evaluations and discuss with residents for future advancement and works with the program director and program faculty as directed

  • Administer a structured educational experience that residents report adheres to ACGME institutional and program specific requirements. The curriculum must be structured so behavioral health is integrated into the residents’ total educational experience, to include the physical aspects of patient care. Delivers regularly scheduled didactics lectures as part of each programs’ didactics curriculum.

  • Participate in the selection process of matching resident candidates to the assigned residency programs. Serve as a member of the Clinical Competency Committee (CCC) for all residency training programs. Attend and actively participate in regularly scheduled faculty meetings, retreats, and development curriculum. Assist in create of faculty development curriculum as directed by the Program Director.

  • LCSW license
  • Arizona Fingerprint Clearance Cards
  • Experience in outpatient counseling and patient assessments/ treatment plans

Preferred Experience:

  • Experience with EPIC Electronic Health Record or similar electronic record systems
